It is authentic Toto.
Did not come with instructions so it has been kind of trial and error in getting an ideal result. My plumbing pipes briefly shake when faucets/cold water are turned on and the flapper needed to be installed a certain way to prevent leaking water and shaking pipes. After installing and periodically hearing shaking pipes I realized the connecting chain was too long and links were getting underneath the flapper after flushing. I needed to remove some links and now it seems to be working as intended.Did not come with instructions so it has been kind of trial and error in getting an ideal result. Thought it might be a good deal, but it isn't.....
Still leaks as badly as my original 12-year old Toto flapper. And, yes, the mating plastic sealing surface is VERY clean. I am going to go with the replacement flapper (with a new part number) from Toto now.

Looks real to me!
These work better than the red Korky ones from Home Depot in a Toto. That's the full review. They also last longer. More expensive but on a cost per effective flush basis I doubt they are

good for the price
Good product for the price . However the rubber is too thin and light and therefore does not seat 100% resulting in a bit of seepage of water . Nonetheless with continued usage it is now seating better. The chain could also have been about 1/4 of an inch longer . I corrected that by adding a longer pin to the chain .

Works just fine, so far.....
Installs easily and so far it works as desired. Basic design is flawed however (as was my previous one). Chain eventually rips the attachment point on the top apart. Not much the supplier can do about it. It is a product design problem.
