Contacted the seller and they immediately shipped out a replacement. The replacement arrived with no issues. Very happy with my purchase. One downside is the label indicates-5v on the output but it doesn't actually work. Not an issue because I didn't need it but others that have older arcade games may need that function. Great value if you are only needing +5V and +12V.
